INTRODUCING
SB/A Core Health Individual Plans
Welcome to the SB/A Core Health Plan. The SB/A Core Health Individual plan takes it cues from the small business Employer plans. The design has a familiar feel as more traditional.
Design features include affordable premiums, typically 50% less than Exchange or marketplace pricing, zero deductible, and generally a 50/50 coinsurance depending upon plan choice.
No medical exams or applications.
The brochure Summary of Coverage plan choice of A, B, or C will determine the amount of (PPO) Preferred Provider Network discounted benefit included. Plan A contains an Annual Maximum Benefit of $10,000 Individual and $20,000 Family. Plan B contains $20,000 Individual and $40,000 Family, and Plan C contains $20,000 Individual and $40,000 Family however an Extended Benefit of $25,000 Individual and $50,000 Family is included upon exhaustion of the base plan benefit.
Consideration of needs and lifestyle, existing monthly provider expenditures, plus personal tolerance for lower discounted benefits can assist in SB/A Core Health Plan selection.
Preferred Provider Discounting (PPO) of invoices is always available and still applies when outside of the annual discounted benefit amount, however it becomes 100% patient pay.
The SB/A Core Health Plans are Affordable Care Act (ACA) qualified for wellness benefits under ACA guidelines and is paid at 100%.
