Marketplace preference for greater co-pay offerings in plans is combined with daily hospital rates for Inpatient and Outpatient surgeries plus professional services to reduce your family costs while managing claim invoicing. Introducing a daily health care plan that is easy to understand, and utilizes dollar-saving telemedicine by Teledoc Health to reduce cost anxiety and handle unforeseen care issues. Careful use of unlimited co-pays can further reduce out-of-pocket expenditures.
Providers include Primary Care Physician, Pediatrics, Internal Medicine, Specialists, and Urgent Care. Inpatient and Outpatient Hospital, Surgical, and Professional Services payments utilize daily rates. Choose $1000, $2000, or $5000 daily rate for hospitals.
Pharmacy needs are provided in the plan by Serve You Rx and their nearly 70,000 nationwide pharmacies. Mental Health benefits are included.
How do I access these health plans?
- Step 1 – Complete and submit the Independent Affiliate Enrollment Packet with DocuSign.

- The applicant completes the SB/A CoOp Membership Agreement and becomes a member of the SB/A CoOp.
- The applicant completes the Independent Affiliate Agreement. Any current 1099 affiliation is approved. Complete the W9.
- The applicant, if not a current 1099 contractor and Affiliate, may become an Advanced Cyber Security (ACS) self-employed contractor (1099). Complete the W9.
- The SB/A Affiliate gains access to SB/A Core Health Plans and Freedom ICON Plans.
- The SB/A Affiliate gains personal access to the Advanced Cyber Security (ACS) suite of products and services.
- The SB/A Affiliate may market Advanced Cyber Security (ACS) products and services.
