The Dental Plans

Affordable and comprehensive Dental/Vision coverage is now available with new dental and vision plan options. We offer a traditional PPO model using DenteMax’s nationwide dental professionals  – the plan covers exams, X-rays, fillings, crowns, bridges, and orthodontia, all the services members require.

For an even more affordable coverage option, we offer a reimbursement plan that covers dental and vision services up to $1800 per year.  Patients can use the dental and vision providers of their choice and be reimbursed amounts based on co-insurance levels.  This is the dental and vision program that employers and members have been asking for – it’s affordable and allows members to keep the dental and vision providers that have come to trust.

How do I access these health plans?

  • Step 1 – Complete and submit the Independent Affiliate Enrollment Packet with DocuSign.
    1. The applicant becomes a member of the SB/A Independent Affiliate Association with the SB/A CoOp.
    2. The applicant, as an Affiliate) becomes an Advanced Cyber Security (ACS) self-employed contractor (1099).
    3. The SB/A Affiliate gains access to SB/A Core Health Plans and Freedom ICON Plans.
    4. The SB/A Affiliate gains personal access to Advanced Cyber Security (ACS) suite of products and services.
    5. The SB/A Affiliate may market Advanced Cyber Security (ACS) products and services.
