Small Business Agency Cooperative is introducing a unique design feature into the SB/A ICON plans for the budget minded Individual looking for a more economical solution in a health care plan.

The SB/A ICON plans incorporate co-pays for Primary Care, Specialist and Urgent Care visits. Plan pays the balance.

The plans incorporate an Emergency Room co-pay of $250. Plan pays balance up to $1000.

The SB/A ICON plans provide scaled up amounts to Outpatient Surgery, Inpatient Surgical Hospitalization and Professional Services, plus Mental Health coverage payments. Scaled up amounts are based on plan selection. ICON I reimburses hospital up to $1,000 per admission, ICON II reimburses up to $2,000 per admission, and ICON V reimburses up to $5,000 per admission.

ACA qualified, Minimum Essential Coverage benefits for routine well care are paid at 100%.

The SB/A ICON plans are an In-Network National Preferred Provider Organization (PPO) with PHCS Network.

Teladoc Health is provided 24/7/365 for direct conversation with Board Certified Physicians.

The design price point contains no maximum out of pocket when outside of the plan footprint.
